Considerations for Older Adults

Plain-language guidance on safer use of CBD and related cannabinoid products in later life. Built around drug interactions, dosing considerations, and the conversation to have with your healthcare team.

Talk to your provider first. If you are taking any prescription medication, do not start a CBD product without speaking with your physician or pharmacist. This is particularly important after age 60, when most adults are managing more than one medication and the body processes medications differently than it did in earlier decades.

Why this matters more in later life

Three things change with age that affect how cannabinoid products interact with the body:

  • More medications, more interactions. Most adults over 65 take multiple prescription medications. Each additional medication introduces another opportunity for interactions. CBD is processed by the same liver enzymes that process many common prescription drugs, which means it can change how those drugs reach your bloodstream.
  • Liver and kidney function shift. Both organs work more slowly with age. Medications and supplements often stay in the body longer than they did in younger years. The same dose can have a bigger effect.
  • Sensitivity goes up. Side effects like drowsiness, dizziness, and changes in blood pressure are more pronounced in older adults and carry higher real-world risks like falls.

The drug interaction picture

CBD is metabolized in the liver by enzymes in the cytochrome P450 family. The most relevant enzymes are CYP3A4, CYP2C19, CYP2C9, and CYP2D6. These same enzymes process a long list of prescription medications. When CBD competes for those enzymes, the blood levels of other medications can rise or fall in ways that matter clinically.

Common medication classes to discuss with your provider

This is not a complete list. It is a starting point for a conversation with your prescriber or pharmacist. Many of these medications are commonly prescribed in later life.

  • Blood thinners (warfarin and similar)
  • Anti-seizure medications
  • Some statins used for cholesterol
  • Some heart-rhythm medications
  • Some blood pressure medications
  • Some immunosuppressants
  • Some sedatives and sleep medications
  • Some opioid pain medications
  • Some antidepressants and anti-anxiety medications
  • Some thyroid medications

Liver enzyme considerations

Peer-reviewed research has documented that CBD use, particularly at higher doses, can be associated with elevations in liver enzymes. This is more relevant for adults already taking medications known to affect the liver, those with existing liver conditions, or those using higher-dose CBD products. Your provider can decide whether periodic liver function monitoring makes sense for your situation.

  1. A complete list of every prescription medication you take, including dose and how often.
  2. A complete list of every over-the-counter medication and supplement you take, including the product name and dose.
  3. Any conditions affecting your liver or kidneys.
  4. The specific product you are considering, including the cannabinoid content per serving from the product label.
  5. Your reason for considering it. Your provider can help you think about whether the product is reasonable for what you have in mind, and what to monitor for.
  6. Whether your provider wants periodic liver function checks if you proceed.

Quality matters more for older adults

CBD products on the market vary significantly in quality. Independent research has documented mislabeling, contamination with heavy metals, and unexpected THC in commercial CBD products. For older adults, these quality issues carry higher stakes: an unexpected ingredient or higher-than-labeled potency can interact with medications in unpredictable ways.

When choosing any CBD product, look for:

  • A current third-party certificate of analysis (COA) from an ISO/IEC 17025 accredited laboratory
  • A batch-specific COA that matches the lot number on the product you are buying
  • Documented testing for cannabinoid potency, residual solvents, pesticides, heavy metals, and microbial contaminants
  • Verifiable compliance with the federal less-than-0.3-percent delta-9 THC limit
  • A clearly identified manufacturer with accessible contact information
